Community Health Representative ECHO Program

The Indian Country Community Health Representative (CHR) ECHO provides comprehensive information to support Indian Health Service, Tribal, and Urban Indian Community Health Representatives and community-based health workers. The ECHO aims to increase access to care for American Indian and Alaska Native people by providing skills of health care provision, disease control, and prevention and help to eliminate health disparities by supporting the workforce to improve Social Determinants of Health (SDOH) using a community-based approach. The program will offer a free ECHO collaborative to continue learning, knowledge sharing and support during virtual ECHO clinics focused on relationship and trust-building, communication and social determinants of health. 2025 topics include three series – Syndemic Approaches (January-April), Specialty Roles Within Healthcare (May-August), and CHR Integration Key Strategies and Best Practices for Improving Care Coordination and the Care Team Experience (September-December):

  • HIV and Hepatitis C (January 13th) – Brigg Reilley and Andrew Yu
  • Sexual Health (February 10th) – Jessica Leston and Niki Graham
  • Congenital Syphilis (March 17th) – Jessica Leston
  • Substance Use Disorder and Harm Reduction (April 21st) – Jessica Rienstra
  • Patient Navigation – Cancer (May 19th) – Alberta Becenti
  • Elder Health Support – Risk Reduction and Caregiver Coaching (June 16th) – Jolie Crowder
  • Immunizations (July 21st) – Holly Van Lew
  • Health Coaching (August 18th) – Ashley Barton and Lucinda Charleston
  • Care Teams/Roles (September 15th) – Samantha Sabo and Louisa O’Meara
  • IHS CHR Care Coordination Demonstration Pilots (October 20th) – Michelle Archuleta, Samantha Sabo and Louisa O’Meara
  • Communicating Across Boundaries (December 15th) – Samantha Sabo and Louisa O’Meara

The 1-hour telehealth sessions will offer community-based health care workes an opportunity to engage with peers and an interprofessional team of specialists working in Indian Country, and an opportunity to obtain a Certificate of Completion following participation in a didactic presentation. The sessions take place virtually using the Zoom platform and are held the *3rd Mondays of every month at 12pm PT starting in January 2025. *Please note due to the holidays, the January and February 2025 sessions will be held on the second Monday of the month at 12pm PT
