In this presentation, Ashley Hoover, MPH, highlights best practices for responding to the rise of syphilis in Indigenous communities and the intersection with substance use disorders. First, Ashley provides an overview of syphilis (including the natural history of untreated syphilis, primary/secondary/latent/tertiary and congenital syphilis, as well as the neurologic manifestations of syphilis), the current epidemiology, the Indian Health Service Chief Medical Officers recommendations, testing algorithms including non-treponemal and treponemal approaches, rapid/point-of-care tests available, as well as testing options for I/T/U clinics to implement (via standing orders, express STI testing services, in-home specimens collection, and expanded screening). Then, Ashley reviews the management and treatment of syphilis & congenital syphilis, post-treatment follow-up, and field based screening and treatment strategies, DoxyPEP, how our EHRs can be leveraged to improve screening, case management approaches, and discusses how we can communicate with and provide incentives to our communities to best respond to the rise in syphilis in Indian Country. Finally, Ashley discusses an opportunity to provide comprehensive clinic assessments, which include interview-style assessments covering topics related to the STI/HIV/HCV and SUD syndemic.

Ashley Hoover, MPH, currently serves as the Syndemic Support Director at the Northwest Portland Area Indian Health Board. Previously, Ashley served as a STD/HIV Perinatal Surveillance Supervisor at the Louisiana Department of Health STD/HIV Program and Research Assistant at the Mary Amelia Douglas-Whited Community Women’s Health Education Center. Ashley has a Master of Public Health in Maternal and Child Health from Tulane University and a Bachelor of Science in Youth Development, International Humanitarian Assistance, Nonprofit Organization Leadership from the University of Florida.
